RUGBY LIABILITY FORM

In order to play with the team you need to renew our registration forms! We need everyone to submit liability release forms on Campus Connect. The form will ask you some personal information, your insurance provider, and for an emergency contact. We’ve attached what you will see once you sign onto campus connect with your umail/CIS login. If you don’t fill this out, you will NOT be allowed to play this season! So make sure you submit it ASAP. The url is https://getinvolved.utah.edu/forms , and your login is what you use to log into canvas. When you fill it out, please select “rugby - women’s” as the club you are joining. If you are not a U of U student, we will have additional info for you later, but you don’t need to fill this out right now. Please let us know if you need help!
